PLE provider renewal
Providers must renew their authority every 12 months. Our office will mail a provider renewal letter with questions and a current list of approved instructors. The provider must answer the questions, sign the letter, indicate any instructor changes and return the letter to the OIC at least seven (calendar) days prior to the expiration date.
Renewal notices are mailed to the provider approximately 40 days prior to the expiration date.
Instructors may be cancelled yearly on your renewal notice, but new instructors must be approved before they can teach a course.
Renewal letters should be returned to the OIC via fax, email or postal mail to the attention of "education analyst."
