Below is a list of some health plans and their provider information. If you don't see a provider on this list, contact the plan directly.
Medicaid is a federally- and state-funded health program that provides care people for people who qualify, primarily due to low income.
For a list of providers who accept Medicaid, call the Medical Assistance Customer Service Center at 1-800-562-3022.
Healthy Options is a medical program that provides no-cost health care services for people on Medicaid or who receive Temporary Assistance for Needy Families (TANF).
