The Uniform Certificate of Authority Corporate Amendments Application is designed for use by an existing insurer for requesting amendments to its Certificate of Authority.
The Uniform Certificate of Authority Corporate Amendments Application can be used to file more than one change in the same submission. The applicant should mark all changes being filed on the application form and submit all items required for those changes in one package.
How to apply
All corporate amendments applications may be filed using the NAIC Uniform Certificate of Authority Application (UCAA).
We encourage filing applications electronically as we have no additional “hard copy” requirements.
State-specific information
We will need the original Certificate of Authority returned for the following:
- Name changes
- Adding or deleting lines of business
- Redomestication
- Change of city or state
- Merger (for non-survivor)
