Update your email or postal address
Log into your account and select "Change Address." If you're updating your email, look for the email address listed in the “Mailing Address” section.
There are no fees for these changes.
If you need your email address updated to reset your password, call, fax or send us a message.
Name change
Individual and business-entity licensees have slightly different submission requirements for changing a name on their license. There is no fee to make a name change.
Steps for changing a name on an individual license
Log into your account and select "Change Name." Designations or titles (like PhD or CLU) cannot be added to your license. You will need to provide one of the following documents that displays your new name:
- Certificate of marriage issued by a county court (certificates from churches or chapels, drivers licenses or passports are not acceptable)
- Court order
- Divorce decree
- Social Security card

Log into your account and select "Change Name."
You'll need to submit additional documentation based on your organizational structure.

Non-resident corporation | A copy of the amended articles of incorporation from your home state showing both names with official stamped approval and a copy of your home-state license reflecting your new name. |
Adding a DBA
Individuals and business-entity licenses can have a DBA added by following these instructions.
- Register the DBA with the Department of Revenue, Business Licensing Service.
- Log into your account and select "Change DBA." There is a $5 fee.
- Provide a copy of an approved registration from the Department of Revenue, Business Licensing Service.
Make corrections to your license information
If you'd like us to correct your name, date of birth, or Social Security number on your license, please contact us and we'll make sure it gets fixed.
