Before you apply

If you are applying to issue discount plans to your customers, it is your responsibility to understand RCW 48.155 RCW, WAC 284-155 and other applicable statutes. Health care discount plan applicants must establish a website to comply with RCW 48.155.070(2) before receiving their license.

Registered discount plan organizations have legal, financial and reporting requirements. Review carefully what's required of you as a discount plan organization. 

How to apply

  1. Download and complete the discount plan organization application (PDF 534.22KB) and gather all the requested documents including the required bond form (PDF 89.11KB).
  2. Print your completed application and have it signed by an authorized officer and notarized.
  3. Combine the application, supplemental documents and any cover letter into a single PDF document. 
  4. Attach your PDF and submit your application via email.
  5. Submit your $250 application payment to one of our mailing addresses.

We acknowledge applications upon receipt.

A non-refundable application fee of $250 is required with your application. You can send your payment to:

USPS will only accept the PO Box mailing address and does not allow other shippers to use that address. All non-USPS shippers must use the street address. Use of an incorrect address may result in a returned application.

After you apply

Applications are assigned to our analysts in the order they're received. Once assigned, an analyst will reach out to the contact listed in question 9 in your application. If there's someone else who should be contacted, be sure to include their contact information in your application. 

We have 90 days from the submission of a complete application to either issue a license or deny the application. A complete application means that all questions have been fully answered, all required documents have been submitted and any deficiencies have been corrected.

If you meet all requirements and any errors in your application are resolved within 15 business days, we'll issue your license by email. 

Licenses are effective for a year and expire on July 1 unless renewed, suspended or revoked before the expiration date.

Once licensed, discount plan organizations have specific reporting requirements. Under RCW 48.155.100, you must notify tus at least 30 days before making any of the following changes to your organization:

  • Legal name 
  • Domicile address 
  • Principal business address 
  • Mailing address 
  • Toll-free telephone number 
  • Internet website address 

Additionally, you should report changes to your company contact to us as soon as possible. 

Renewal and annual reports

Renewal applications and a $200 renewal application fee are due at least 90 days before a discount plan organization's license expires. See RCW 48.155.020(8) and WAC 284-155-015(2) for complete details.

Annual reports must be submitted by March 31 each year. If you include your annual report with your renewal application, you do not need to pay the $20 filing fee. If you need additional time to prepare your audited financial statements, you must request an extension at least ten business days before the filing deadline.
