Who needs to submit information

If you have sold, plan to sell, or have an approved filing for a fixed payment policy in Washington state, you need to complete our survey.

Due date

You need to submit your survey responses by February 28 each year. We send the report to the Legislature by June 1.

How to complete the survey

Each year, we'll send you a letter asking for information about your activities in the fixed payment insurance marketplace. You need to complete the survey using our Excel template.

Email your information to the address in the Washington Fixed Payment Instructions below. Include the name, email address and phone number of the person we should contact about this report. We'll send them future requests. If this information changes, contact us at the email address in the instructions.

What counts as a fixed payment policy

For this report, we consider the following to be fixed payment policies:

  • Illness-triggered fixed payment insurance
  • Hospital confinement fixed payment insurance
  • Other fixed payment insurance policies that pay a fixed dollar amount regardless of what they charge

This doesn't include:

  • Fixed payment riders for any policy.
  • Policies we usually define as disability income policies.
