The Balance Billing Protection Act directs the Office of the Insurance Commissioner (OIC) to develop a list of approved arbitrators or entities that provide arbitration for balance billing disputes.
If the insurer and provider or facility cannot agree on the amount for the service within 30 days, the dispute is settled through arbitration.

Arbitrators must issue their final decisions within 30 days of the parties’ written submissions.
This settlement reporting form is required by the Office of the Insurance Commissioner (OIC) for parties who have settled proceedings under RCW 48.49.040. This mandatory form is referred to as “Appendix D” in WAC 284-43B-100. Submission of this form satisfies the reporting requirements under RCW 48.49.040(7).
