Treatment limits

  • Limits on the number of days or visits
  • Other limits on the amount or length of treatment
  • Insurer approval for services (prior authorization)
  • Medically necessary services
  • Requirements to try cheaper medications first (step therapy)
  • Provider reimbursement rates
