Why does my insurance company want to change its rates?

We created summaries in the health rate search tool to help you understand why companies believe they need to change rates. The data in the summaries is supplied by the company. You can find summaries for each request that include:

  • The average percentage change the company is requesting
  • When new rate takes effect
  • The number of people impacted
  • Information about how the company developed its rate
  • How the company will spend your premiums
  • Changes to benefits
  • The company's rate request history

To see a summary for any rate request:

  1. Go to the health rate search tool
  2. Click the dropdown and find the company name you're looking for
  3. Look under "Request details"

Part of our rate review includes double-checking and verifying the company's information. If we disagree with the company's data, we may deny the rate change. Learn more about how we review health rates

How can I find out more about your decision to approve or deny a rate request?

Once we've reviewed the rate request and made our final decision, we write a decision memo. Decision memos include:

  • Our reasons behind the decision to approve or deny the rate request
  • What we considered in our review
  • Key information the company used to develop its request
  • If we denied the rate request, why and because of what factors
  • Any changes to benefits.

How can I find out when my company wants to changes my rate?

You can sign up to get email notifications whenever your health insurer wants to change your rate. Once you've signed up and selected your company, we'll send you an email notification whenever:

  • Your health plan requests a rate change
  • The request is withdrawn
  • The request is revised
  • We've made our decision

Minimum qualifications include:

  • Licensed to do business in the State of Washington or provide a commitment that it will become licensed in Washington within thirty (30) calendar days of being selected as the Apparent Successful Contractor.
  • Demonstrate knowledge of, and prior experience in, the areas of work for which the proposal is being submitted. Vendor must have previously held three (3) years of responsibilities substantially the same as, or very similar to, the Scope of Work in Section 1.2 of this RFP.
  • Contractor must be or employ actuaries who are members of the American Academy of Actuaries and Society of Actuaries and meet the qualification standard to complete the scope of work stated in this RFP.
    • Credentialed actuaries shall perform or be responsible for all actuarial services under the Contract. Actuarial work must be supervised by an individual who is a member of the American Academy of Actuaries. 
  • Agree to the Certifications and Assurances set forth in Exhibit A.
  • Submit proposals as specified on Section 3, Proposal Contents, of this RFP.

Company Complaint Response System (CRS) help

We created the Company Complaint Response System (CRS) to securely communicate with insurance companies online. CRS replaces the need to mail, fax, or email complaint-related documents while still keeping consumers' private information safe. However, CRS does not handle your company’s internal processes for investigating, responding to, and resolving consumer complaints that you receive from our office.

Company registration

Companies with a current WAOIC number and a designated primary contact for complaints are automatically registered. To register, contact our office and provide the following information:

  • Company name
  • Contact name
  • Complete mailing address
  • Telephone number
  • Fax number
  • Email address 
  • NAIC number
  • WAOIC number

Once you register, you will be emailed user ID and temporary password, along with general information about using the system.

Login, User ID and password

How to log in

To log in, go to the CRS login page, enter your user ID and password, and click "Go."

You will receive an email with a link to log in when a new complaint or document is added to CRS, .

User ID

  • Primary complaint contact user ID is "CRS" plus your company’s WAOIC# (e.g., CRS500846).  
  • Secondary user IDs follow the same format, with a dash and a sequence number (e.g., CRS500846-1, CRS500846-2, etc.).

Note: Secondary users who don’t know their user ID must obtain it from their primary complaint contact.

How to recover a password

  1. Click “Forgot Password?” on the main CRS login page in the user login box.
  2. Enter your user ID and email address.
  3. Click "Submit."

You will get an email with a temporary password. After you log in for the first time, you will be prompted to change your password.

How to change your password

  • After you log in, select "Change Password" from the “Menu Options” in the left-hand navigation
  • Enter your old password.
  • Enter your new password.
  • Confirm your new password by entering it again.
  • Click “Submit.”
Primary complaint contact

The primary complaint contact is the person at your company who communicates with us about consumer complaints. This person is responsible for:

  • Coordinating responses to consumer complaints.
  • Updating the primary contact information, including your company's main contact person.
  • Mainting the list of secondary users and answers any questions they have about the CRS and their user IDs.

How to update the primary complaint contact information

  1. In the left-hand navigation, under “Menu Options,” select “Update Contact Information.”

  3. Complete all required fields and click "Submit."

How to find the primary complaint contact

  • Go to the CRS login page
  • Under "Find your company complaint contact" enter the WAOIC number or the NAIC number
  • Click “Find.” The contact information will appear below.
Secondary user

A secondary user is an insurance company employee who is granted access to CRS by the primary complaint contact. Secondary users:

  • Get their own user ID and password.
  • Receive automatic emails when a new complaint or document is added to CRS.
  • Work with the primary contact to find answers to questions and update their status or contact details.

How to add a secondary user

  1. In the left-hand navigation, under “Menu Options,” select “Secondary User.”
  2. Click “Add Secondary User.”
  3. Complete all mandatory fields and click "Submit."

How to delete a secondary user

You cannot delete secondary users from the list. However, you can change their status to “Inactive,” as shown here:

How to update secondary user contact information

Only the primary contact person can update a secondary user’s contact information.

Documents 

How to upload documents

Click the link for the case number, then select "Upload New Document." Choose the PDF document you want to upload from your computer. Once your document has been successfully uploaded, click "Finished."

You will not receive an email to confirm that the OIC received your uploaded document. However, you can see that it has been received when it shows up in the “Document Name” column.

Helpful Tips:

  • Keep documents under 100 megabytes (MB).
  • Ensure your documents show up in the "Successfully Uploaded Documents" area before clicking “Finished.”
  • Avoid uploading pictures. If you're unsure, ask the analyst assigned to your case for advice.

Documents cannot be deleted by users

If you need a document deleted, send us a message with the:

  • Specific case number
  • Name of the document
  • Reason you want it deleted

I got an email about a new document for a case, but I can't find it in "Open Complaints." What should I do?

Documents like the "Closure Notice" may be uploaded after a case is closed. If you can't find the case in "Open Complaints," please check "Closed Complaints."

Once a case is closed, it is moved to the “Closed Complaints” section in CRS. If we decide to reopen a case, it moves back to the “Open Complaints” section. Cases stay in CRS for one year from the last “Closed Date.”

Received a complaint for another company

If your company receives a complaint meant for another company, follow these steps:

  1. First, check if the complaint was sent to the correct WAOIC#.
  2. If not, upload a response in CRS with that information.
  3. Once the complaint is corrected, we will remove it from your company’s “Open Complaints.”
Email notifications

The primary complaint contact will receive an email from the OIC when:

  • We first register your company with CRS
  • You add a secondary user
  • A new complaint has been added
  • A new document has been added
  • Your password is reset
  • Your primary complaint contact information is updated

Secondary users will receive an email from the OIC when:

  • A new complaint has been added
  • A new document has been added
  • Your password is reset

Issues with receiving emails

  • Check your email program's spam or junk folder.
  • If you are a secondary user, verify if the primary complaint contact has changed your account status to "inactive."
  • If you share an email address with others, please note that we will only send emails to one address.

Emergency Rule: Plan Year 2026 development of uniform cost-sharing reduction factor (R 2025-01)

The Office of the Insurance Commissioner (OIC) must review and approve all individual health plan rates prior to their use. This emergency rule adjusts health insurer rate development components for Plan Year 2026 to preserve consumer affordability and stability in Washington state’s individual health insurance market. It applies a uniform cost-sharing reduction silver load adjustment factor to rates for silver level qualified health plans (QHPs) sold on the Washington state Health Benefit Exchange.

Premium tax filing instructions for surplus line brokers 2024

New for 2024

There are no changes from the 2023 surplus line broker tax forms.

Filing requirements

All business entities with a surplus line broker license and unaffiliated individuals with a surplus line broker license in the State of Washington during the tax year must complete a form, even if no business was actually transacted within Washington during the tax period. Please review surplus line broker reporting and tax requirements if you are not sure if you need to file.

Due date

The completed online form and payment must be received by the our office on or before March 1 following the end of the calendar year. When the due date falls on a Saturday or Sunday, the form and payment are considered timely if received the next business day.

 Caution: Do not mail a paper version of the form or any attachments to our office.

Completing your premium tax form

The tax rate is 2%. Calculations are completed automatically.

The Surplus Line Association of Washington (SLA) enters premiums on your behalf as a courtesy; your tax form will not be available to file until they have entered your premiums. The tax contact OIC has on file for your entity will receive an email notification when your premiums have been entered.

The surplus line broker tax form is read-only.

 Important: It is your responsibility as a surplus line broker to ensure your premiums are entered correctly. If you don't agree with the premium amounts entered on the tax form, contact SLA of Washington.

Declaration and final submission

Review the declaration and, if necessary, update the contact information.

The contact information listed here is the insurance commissioner's only point of contact for premium tax purposes; please make sure the information is accurate.

Enter the name and title of the officer that has examined the form and declares that to the best of this officer’s knowledge and belief, the information is entirely true, accurate and complete.

Submit your form to the insurance commissioner by clicking "Final Submission."

Premium tax filing instructions for risk retention groups 2024

New for 2024

The risk retention tax form changed to reflect updates NAIC made to Schedule T:

  • Inland Marine line changed from 9 to 9.1
  • Added line 9.2 for Pet Insurance Plans

The Office of Insurance Commissioner (OIC) provides a resource to aid foreign companies in retaliatory reporting. The list is not all-inclusive, but does provide common fees, assessments, and surcharges that must be reported.

Premiums and Annuities - Additional Detail

Premiums reported in this section are not in addition to premiums reported in the previous section; they are a subset of those premiums.

The purpose of breaking out the premiums is to allow us to accurately apply taxes and tax exemptions to the proper premiums. Premiums reported in this section capture detail not available on the Life Insurance Part 1 and Accident and Health Insurance pages of the Annual Statement. These premiums are carried through to the appropriate schedules.

Non-Exchange Premiums

  • Qualified Life premiums are certain products issued that are exempt or qualified under certain sections of the internal revenue code.
  • Qualified Life Dividends are from certain products issued that are exempt or qualified under certain sections of the internal revenue code.
  • Qualified Annuity premiums are certain products issued that are exempt or qualified under certain sections of the internal revenue code.

  • Adult Dental premiums entered are used to reduce the taxable premiums on Schedule C.
  • Qualified Pediatric Dental Kids Only (Non-Exchange) premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Individual Market premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Small Group Market premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Large Group Market premiums entered are used to reduce the taxable premiums on Schedule C.
  • Non-Qualified Pediatric Dental premiums entered are used to reduce the taxable premiums on Schedule C.

Exchange Premiums

Tax revenue from all WA Exchange premiums is required to be deposited into an account separate from other tax revenue at the state treasury.

  • Qualified Health Plan premiums reflect plans offered through the WA Exchange and are taxed on Schedule D.
  • Adult Dental premiums reflect plans offered through the WA Exchange and reduces the taxable premium on Schedule D. In 2016, no stand-alone adult dental plans were offered through the Exchange.
  • Qualified Pediatric Dental Kids Only premiums reflect plans offered through the WA Exchange and are taxed on Schedule D.
  • Qualified Stand-Alone Family Dental - Individual Market premiums reflect plans offered through the WA Exchange and are taxed on Schedule D.

New for 2024

Health company tax forms changed to reflect updates NAIC made to Schedule T:

  • Vision Only and Dental Only swapped places on the tax form.

Health company tax forms were updated to add two new lines under "Health Premiums - Other"

  • Inpatient Direct Payments (Received from HCA)
  • Hospital Safety Net Payments (Received from HCA)

Filing requirements

All health maintenance organizations, health care service contractors, and self-funded multiple employer welfare arrangements licensed in the State of Washington during the calendar year must file an online tax form, as provided by this office, even if no business was actually transacted within Washington during that time. The filing of any form other than the online one provided by this office is not accepted.

Due date

The completed online form and payment must be received by the Office of Insurance Commissioner on or before March 1 following the end of the calendar year. When the due date falls on a Saturday or Sunday, the form and payment are considered timely if received on the next business day.

Caution: Do not mail a paper version of the form or any attachments to our office.

Completing your premium tax form

Tax is generally due on all premiums, unless specifically exempted on the tax form. If you have questions, please contact us.

Premiums reported must be on the same basis as required on the Annual Statement.

The tax rate is 2%. Calculations will be completed automatically.

Selecting "Print" will print your form exactly as it appears on the screen, even if you have not saved your work.

The "Save Progress" button does not check for errors.

Remember: Save your work before moving away from the form; your work is not automatically saved.

Premium Reporting section

Health Premiums Written as Reported on the Annual Statement

All premiums and prepayments collected or received must be reported in this section.

Premiums reported in this section should, with few exceptions, match premiums reported for Washington on the Exhibit of Premiums, Enrollment and Utilization filed with the National Association of Insurance Commissioners (NAIC).

Health Premiums - Other

Medicaid pass-through payments are received from Washington State Health Care Authority and must be reported in this section.

ACA Risk Adjustments/Risk Corridor Transactions are payments paid or received. These transactions are not considered premiums for tax filing purposes.

  • If your company paid Washington-related risk adjustment or risk corridor transactions, enter the amount as a positive number. This will reverse the amount reported on your annual statement that resulted in a premium reduction.
  • If your company received Washington-related risk adjustment or risk corridor transactions, enter the amount as a negative number. This will reverse the amount reported on your annual statement that resulted in a premium increase.

Other (Specify) is to report premiums other than those reported on the lines above.

Health Premiums - Additional Detail

Premiums reported in this section are not in addition to premiums reported in the previous section; rather, they are a subset of those premiums.

Premiums reported in this section capture detail not available on the Exhibit of Premiums, Enrollment and Utilization. These premiums are carried through to the appropriate schedules as appropriate.

Non-Exchange Premiums

  • General Assistance - Unemployable (GAU) premiums are payments received for services provided under RCW 74.09.035. Premiums entered are used to reduce the taxable premiums on Schedule A.
  • Basic Health Plan (BHP) (Subsidized premiums) are premiums received on behalf of subsidized enrollees as provided in RCW 70.47. Premiums entered are used to reduce the taxable premiums on Schedule A.
  • Adult Dental premiums entered are used to reduce the taxable premiums on Schedule A.
  • Qualified Pediatric Dental Kids Only premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Individual Market premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Small Group Market premiums are for informational purposes only.
  • Qualified Stand-Alone Family Dental - Large Group Market premiums entered are used to reduce the taxable premiums on Schedule A.
  • Non-Qualified Pediatric Dental premiums entered are used to reduce the taxable premiums on Schedule A.

Exchange Premiums

Tax revenue from all WA Exchange premiums is required to be deposited into an account separate from other tax revenue at the state treasury.

  • Qualified Health Plan premiums reflect plans offered through the WA Exchange and are taxed on Schedule B.
  • Adult Dental premiums reflect plans offered through the WA Exchange and reduces the taxable premium on Schedule B. In 2016, no stand-alone adult dental plans were offered through the Exchange.
  • Qualified Pediatric Dental Kids Only premiums reflect plans offered through the WA Exchange and are taxed on Schedule B.
  • Qualified Stand-Alone Family Dental - Individual Market premiums reflect plans offered through the WA Exchange and are taxed on Schedule B.
  • Qualified Stand-Alone Family Dental - Small Group Market premiums reflect plans offered through the WA Exchange and are taxed on Schedule B.
  • Qualified Stand-Alone Family Dental - Large Group Market premiums entered are used to reduce the taxable premiums on Schedule B.
